I'm Jess, a local, experienced occupational therapist. I help veterans, war widows and widowers keep living safely in their own homes, with falls prevention, home modifications and equipment, all funded through DVA.

Three simple steps
If you hold a DVA Gold Card, or a White Card for an accepted condition, you may be eligible for OT home visits funded by DVA.
See your GP
Ask your GP for a D904 referral for occupational therapy.
Book your visit
Jess will call you to book a home visit at a time that suits you.

I can recommend and organise home modifications, funded by DVA.

Referring to Jess
- Upload the D904 with the online referral form, or email it to mail@glideconsulting.com.au. Jess will contact the veteran promptly to book.
- Each referral covers a DVA treatment cycle of up to 12 sessions within 12 months.
- You'll receive a clear, one-page summary after assessment and a report at the end of each cycle.
